Reconnect Community Health Services is a not-for-profit health service organization located in the west end of Toronto. We provide services and support for seniors, caregivers, seniors and adults with disabilities, and people living with mental health and addictions concerns to improve and maintain their health and well being.We offer a wide range of programs, including crisis outreach, home help and respite card, meals on wheels, drop-in support groups, one-on-one counselling, and specialized programming for children and youth.
